Meet Sharon, an Anthem patient who was diagnosed with Crohn’s disease and, because of that, suffered 10 hospital readmissions in two months. Sharon’s situation “was touch-and-go there [for awhile],” says her husband, Steve. With help from their Anthem Nurse Case Manager, Gretchen McKinney, the couple was able to understand “what we need to do next to get Sharon better,” Steve says, including home care needs and other resources. Now that Sharon is back on her feet, see the couple’s emotional reunion with Gretchen. “I think I’m a better case manager because of them,” Gretchen says.
